   Instructions
  POSTURE
Work surfaces are adjustable or at optimal heights.
  Wrists remain neutral (straight).
  Neck remains straight.
Bending at the waist is minimized.
Twisting at the waist is avoided.
  Squatting and stooping are avoided.
Arms/Elbows remain close to the body and in neutral position.
  Extended reaching is avoided.
  Raised elbows are avoided.
  Overhead work is avoided.
Contorting the body due to poor clearances is avoided.
Tool handles are the proper size (not too small/large).
  Employees can sit or stand.
  Frozen (static) postures are avoided.
Resting body parts on sharp edges is avoided.
  Cramped work areas are avoided.
Work requiring kneeling (low work) is minimized.
Work is within comfortable reach zone.
Reaches are adequate for smallest workers.
Clearances are adequate for largest workers.

FORCE
Loads are lifted close to the body.
Lifts below knuckle height or above shoulder height are avoided.
Suspect lifts over 10 lbs. are analyzed with NIOSH lifting equ.
Containers have good hand holds. All handles are adequate.
Objects handled are stable; loads don't shift while handling.
Sudden, forceful or unexpected motions are avoided.
Workers are trained in proper lifting techniques.
Pull and Push forces are not excessive. Analyze with push/pull calculator.
  Carried object weight is minimal.
Pull, push, and carry distances are minimal.
Push forces are minimal; Heavy loads roll easily (high quality casters).
One hand push/pulls over 7 kg (15 lb) have been analyzed.
One hand lift/carries over 4.5 kg (10 lb) have been analyzed.
Tasks can be done with either hand or both hands.
Power grip forces are less than 10 lbs.
Pinch grip forces are less than 2 lbs.
Hand tools are counter balanced if tool weight is over 3 kg (7 lb).
  Static muscle contraction/loading is avoided.
Forces required to do job are acceptable to workers.
Mechanical lift, carry, push, pull aids are used when possible.
Assembly fixtures and jigs are available.
A preventative maintenance program for equipment/tools exists.

FREQUENCY/DURATION
  Highly repetitive/monotonous tasks are avoided.
Lifting/pushing/pulling occurs less than once every 5 minutes.
Non-value added handling of loads is eliminated.
Anti-fatigue mats are provided for prolonged standing tasks.
Hand tools are powered when possible.
  Excessive overtime is avoided.
Rest breaks, self pacing, or job rotation is provided.
Work pace && rest breaks are acceptable to workers.
  ENVIRONMENT
  Lighting is adequate and analyzed.
  Glare is minimized.
Individual task lighting is provided where possible.
  Noise levels are acceptable.
  Vibration exposure is avoided.
  Temperature in work area ranges from 21 - 27 C (70 - 80 F).
  Air circulation is present.
CONTROLS && DISPLAY
  Displays are organized and intuitive.
  Related displays are grouped together.
Displays are located next to their corresponding control.
Displays and controls are clearly labeled.
Controls are color coded or shape coded.
Approximate value or rate-of-change displays are analog.
  Precision value displays are digital.
  Non-value added displays are eliminated.
  Control forces are minimal.
Foot controls are used when high forces are necessary.
Top of display height is at eye level or below. Display is adjustable.

OTHERS
  Employees know symptoms/signs of injuries.
Objective measurements of issues were obtained (forces/weights, postures, frequencies, light levels, etc.).
